You can use ASCQ-Me as a paper-and-pencil (PAP) assessment or as an electronic assessment. The PAP includes six individual forms with five questions each, and one form that contains nine questions. The electronic assessment is accessed through a URL where you choose which ASCQ-Me assessment you want and whether you want the assessment administered in a fixed format (every person answers the same set of questions) or adaptively (each person receives a set of questions appropriate to his or her level of health).
